How do I change my address?

If you have recently moved and you need to update your address please contact member services.  Your member services number is located on the back of your card.

I have questions about my premium and eligibility.

If you have questions about your premium or whether or not your policy is active, please call the member services number located on the back of your card.

How do I order a new insurance card?

If you have lost your insurance card, you will need to call member services in to order a new one.  If you have your policy number but cannot find the member services phone number you can call the 800 number listed for your policy below:

What is Multi-Plan?

Multiplan is an organization that works with providers to offer discounts to members.  They work with the provider creating a network which allows you to take advantage of these discounts.
